Please do not submit medical or dental history, symptoms, diagnoses, insurance identification numbers, or other sensitive health information through a form on this site. If you need to discuss your care, call our office at (305) 400-8060.

Our handling of protected health information as a patient of our practice is governed by our Notice of Privacy Practices, not by this website policy. You may request a copy at our front desk or by contacting our office.

When you complete an appointment request or contact form, we collect the information you choose to enter. This may include your name, phone number, email address, preferred appointment day and time, insurance-change information, and any message you choose to write.

We use this information to contact you, confirm or schedule an appointment, and answer your question. If you become a patient, relevant information becomes part of your patient record and is handled under applicable privacy requirements.

3. Information collected automatically

Our servers and permitted tools may collect standard technical information, including your IP address, browser and device information, the pages you view, referring source, and the date and time of your visit. We use this information in aggregate to understand how people find and use the site.

4. Cookies and similar technologies

A cookie is a small text file placed on your device. We provide a Cookie Preferences control so you can choose optional categories before they load.

• Strictly necessary technologies. These keep the website working and remember your privacy choice. They cannot be turned off through our preference panel.
