This website is a public information site. It is not a patient portal and it is not a place to send clinical information. Please do not submit medical or dental history, symptoms, diagnoses, insurance identification numbers, or other sensitive health information through a form on this site. If you need to discuss your care, call our office at (305) 400-8060.
Our handling of protected health information as a patient of our practice is governed by our Notice of Privacy Practices, not by this website policy. You may request a copy at our front desk or by contacting our office.
2. Information you give us
When you complete an appointment request or contact form, we collect the information you choose to enter. This may include your name, phone number, email address, preferred appointment day and time, insurance-change information, and any message you choose to write.
We use this information to contact you, confirm or schedule an appointment, and answer your question. If you become a patient, relevant information becomes part of your patient record and is handled under applicable privacy requirements.
3. Information collected automatically
Our servers and permitted tools may collect standard technical information, including your IP address, browser and device information, the pages you view, referring source, and the date and time of your visit. We use this information in aggregate to understand how people find and use the site.
4. Cookies and similar technologies
A cookie is a small text file placed on your device. We provide a Cookie Preferences control so you can choose optional categories before they load.
- Strictly necessary technologies. These keep the website working and remember your privacy choice. They cannot be turned off through our preference panel.

- Functional technologies. These enable the GoHighLevel appointment form and LeadConnector chat and are disabled unless you select Functional.

When you provide the relevant consent, this site may use Umami for analytics, GoHighLevel for appointment requests, and LeadConnector for live chat. These providers may process technical information needed to provide their services. We do not load analytics technology on the appointment-request page, and we do not use information entered into a form to target advertising to you.

You can revisit at any time, or adjust and clear cookies through your browser settings. We do not currently process Do Not Track or Global Privacy Control signals as separate automated requests; the Cookie Preferences control is the way to manage optional technologies on this site.
